Course Change Form

The course change form is available online through the Registrar's website.

This form is used to:

  1. Add or drop classes after deadlines, listed below
  2. Change course sections or course credits
  3. Change a course to an audit
  4. Add or delete honors enrollment in a course

Adding courses:

Courses may be added by using MyUW according to this schedule:

  • Full semester courses — during the first two weeks
  • Modular courses (usually 4-8 weeks long) — during the first week of the course
  • 8-week Summer Session courses — through the first week of classes
  • Summer Session courses shorter than eight weeks — through Wednesday of the first week of class

Under extenuating circumstances, courses may be added after the deadline with your dean's permission and a course change form. The form must have:

  1. Advisor's signature
  2. Stamp of the department offering the course
  3. Your dean's signature

If adding a course brings your semester credit load above 20, you must get your dean's permission to carry the overload. Completed forms should be turned in to your dean's office.

Dropping courses:

Courses may be dropped MyUW according to this schedule:

  • Full semester courses — during the first nine weeks
  • Modular courses (4-8 weeks) — through the first half of the course
  • Summer Session courses — through the first half of the course

When courses are dropped by the Timetable deadline, no entry will be made on your official transcript. See the Timetable for details.

Under extenuating circumstances, a course may be dropped past the nine-week deadline, with your dean's permission and a course change form.

If dropping a course will bring your semester credit load under 12 credits, you will be automatically placed on probation or dropped (if already on probation) unless you obtain your dean's permission to be a part-time student. The completed form should be turned in to your dean's office.

Credit change:

A few courses offer variable credit. Credit changes may be made only by using the course change form according to the following timetable:

  • Full semester courses — during the first nine weeks of a semester
  • Modular courses or Summer Session courses — during the first half of the course

After those deadlines, you must use a course change form and include signatures from the professor and your advisor. If changing credits brings your semester credit load under 12 or over 20 credits, you must see your dean for permission to change credits. Turn in the completed form to your dean's office.

Audits:

Courses may be changed to an audit with the instructor’s permission during the first four weeks of the course. You must use the Credit Change section of the course change form. Audited courses do not count in the minimum number of credits required in each term.

Section change:

Section changes should be made according to this guideline:

  • Full semester courses — during the first nine weeks of a semester
  • Modular or Summer Session courses — during the first half of the course

Obtain department and/or instructor approval to enter the new section. Your advisor's signature is not needed. Turn in the completed form to your dean's office.
